Tóm tắt : | 39 p. Skill-mix innovations are an important part of the reorganization of care for people with chronic conditions and multimorbidity. With their focus on improving coordination and patient-centeredness they are at the core of many integrated care models and health professionals involved in the new roles, tasks and modes of working are often seen as the true ‘integrators’ of care. This Policy Brief identifies (in terms of improving integration of care of patients with chronic conditions and multimorbidity) six skill-mix innovations which enable: (1) Shifting tasks and roles, (2) Relocation of care to other settings, e.g. to nurse-led clinics, (3) Introduction of care coordination role, (4) Empowering patients and caregivers through new roles, (5) Introduction of dedicated prevention roles in primary care, (6) Establishment of teamwork and collaboration in multi-professional teams. |
