This publication aims to provide practical tools building on the approaches identified in the ‘Healthy Islands: Best Practice in Health Promotion in the Pacific’ to enable Pacific communities as part of on-going efforts to implement strategies to reduce modifiable risk factors for noncommunicable diseases (tobacco use, diet, physical inactivity and harmful use of alcohol), including the underlying social determinants.
