Tóm tắt : | Issues of budget execution delays and rigidities have been identified as an impediment to successful implementation of health financing and purchasing reforms in a number of low- and middle-income countries. This paper is a country case study of health budget execution issues in Kyrgyzstan. Using an approach adopted in other WHO country case studies, it systematically identifies budget execution issues and determines their underlying causes, as a basis for proposing policy responses. It classifies the causes that arise at all stages of the budget cycle: formulation, execution, monitoring, reporting and audit. It also distinguishes causes that arise from the public financial management system and causes that arise specifically in health sector institutions and processes. Kyrgyzstan was chosen as a case study because it has carried out wide-ranging health financing reforms over the past 25 years, including the introduction of output-oriented provider payment methods by the Mandatory Health Insurance Fund, but it has also faced persistent budget execution bottlenecks and rigidities. Kyrgyzstan also initiated a public financial management reform, including steps towards programme budgeting, and piloted these in the health sector. xv, 65 p. |
